Melbourne Street to Home
AimMelbourne Street to Home is a new program providing coordinated housing, support services and health intervention targetted at Melbourne's most vulnerable rough sleepers. Melbourne Street to Home will identify the most vulnerable rough sleepers in Melbourne, help them access safe, secure and affordable long-term housing and provide ongoing support to sustain successful tenancies. The program is a partnership between HomeGround Services, The Royal District Nursing Service and The Salvation Army Adult Services and The Salvation Army Crisis Services. EligibilityEligibility for this program is determined by a Vulnerability Index tool designed to select those most in need of assistance. The Vulnerability Index is administered during a Registry Week each year in which volunteers seek out people sleeping rough in Melbourne. |

Street to Home programs are being implemented in Sydney, Brisbane and Canberra and one is already operating in Adelaide. It is based on a New York model, which successfully reduced rough sleeping by 30% over 12 months.
