Housing Mental Health Pathways Program

Aim

The Housing Mental Health Pathways Program (HMHPP) prevents patients from the Alfred and St Vincent's hospitals' acute psychiatric wards from becoming homeless.

Eligibility

HMHPP targets patients with histories of homelessness who have no suitable accommodation options at time of discharge.

Access

HMHPP accepts referrals from the Alfred or St Vincent's hospitals' acute psychiatric wards.
