ConnectED
Aim
ConnectEDÂ improves quality of life for participating clients by working with clients and community services to provide appropriate and relevant support in the community in which they live. It aims to reduce preventable use of the Alfred Hospital's emergency health services by people who are socially isolated or homeless.
ConnectED is a partnership between HomeGround, the Alfred Hospital Emergency Department, Royal District Nursing Service - Homeless Persons Program, Bentleigh Bayside Community Health Service, Inner South Community Health Service and Prahran Mission.
Eligibility
ConnectED is for people experiencing homelessness, or at risk of becoming homeless, who are frequent users of the Alfred Hospital's emergency department.
Access
ConnectEDÂ accepts referrals from the Alfred Hospital Emergency Department.
