Crisis Support
Outreach workers provide short-term,
intensive, housing-focused crisis support to homeless people in the
cities of Hume and Moreland. This program is an integral part of the
local service system.
Housing
High volume service centres in St Kilda and Collingwood provide housing information and referral services for people who are homeless or at risk of homelessness in the cities of Yarra, Port Phillip, Stonnington and Melbourne.
These are key entry points for people accessing the homelessness service system and carry out an intake and assessment role.
Justice Housing Support
Housing-focused outreach workers
deliver services across metropolitan Melbourne to people represented in
the justice system with histories of homelessness. Referrals are
through Magistrates’ Courts.Workers based at the Sunshine Magistrates’ Court, Melbourne
Magistrates’ Court and at Australia’s first Neighbourhood Justice
Centre in Collingwood deliver housing, information and referral
services.
Outreach and Outreach Support
Outreach workers provide
long-term support to people with complex needs and long histories of
homelessness, including support after people attain permanent housing
in order to help them keep it.
Housing Outreach Worker
The Housing Outreach Worker program is an innovative response to a gap in the service system for the most vulnerable homeless people. HomeGround provides a housing focused outreach service to those living in boarding houses and on the street.
Private Rental Access Program (PRAP)
This program
provides housing-focused assistance to women (and their children) who
are escaping from, or experiencing, family violence to help them access
and maintain long-term private rental accommodation.
Mental health and health programs
Housing Mental Health Pathways
Housing-focused outreach workers provide services to mental health patients from acute psychiatric wards at St Vincent’s Hospital and The Alfred Hospital. This service targets people who have a history of homelessness or no suitable housing options upon discharge.
ConnectEd
This housing-focused outreach program aims to reduce preventable use of The Alfred Hospital’s emergency services by people who are socially isolated or homeless. This position sits within the local outreach health team.
Social Housing Advocacy and Support Program (SHASP)
This program provides support to and advocates on behalf of people living in public and social housing to help them successfully establish and sustain their tenancies.
Tenancy and Property
HomeGround manages over 330
transitional housing properties for homeless people across the Cities
of Yarra, Port Phillip and Stonnington in partnership with 60 other
agencies that provide support to our tenants.
